You might want to take a look into the Racial Substitution levels from "Races of Stone" and the Racial Paragon miniclasses from UA.

Just choose a race, like dwarf or half-orc.

In fact, with Races of Stone, dwarf fighters make very tough Tanks. Exotic Armours, Extreme shields, Armour Optimization feats, the three racial substitution levels (with d12!)....

Go Giant from AU, take the racial levels, some warmain levels, and then Giant Paragon.

This is what I was waiting for. The Warmain is a fighter style class, although highly specialized, who gets more hit dice and can wield larger weapons as they go up in levels.

It's available in Arcana Unearthed, will be in Arcana Evolved, and is in Way of the Sword, a mini-PDF that takes all the martial aspects of Arcana Unearthed into one file.

The Gallowglass is another good option. Very tank like, but also very slow.

The defender class from Book of Exalted Deeds is another good one.
