Why aren't flat chested women ever barmaids?

I blame the Germans.

Trust me, that's tame compared to what the Germans really offer. ;)

What do these flat chested women do for a living?

They're scullery maids. ;)

Men are cool with a fat Half-Orc serving them drinks from behind the bar,

Makes sense. Adventures will go to the rowdy dives, so they want a beefy half-orc, human, or even no-nonsense dwarf running things. You need someone who can crack heads when a fight breaks out. Do you think an ELF will cut it those establishments? I think not!

BTW I've never seen a half-orc barman in any game, nor even any dwarves I can think of. So I'm not sure where that stereotype comes from. And the main tavern in my 'Duskmoon Hills' D&D PBEM, the (ex mercenary) innkeeper's daughter is notably slim and quite flat-chested. He does have one or two buxom serving wenches though. :)

Never a dwarf barman... I mean, everyone would think there was no one behind the bar.

This isn't just restricted to D&D. If you watch a local sports broadcast, chances are good you'll see several commercials for local sports bars, and all of them apparently hide all the less well endowed staff (or maybe don't employ any for all I know)

Actually, it isn't the sexual side of things, its biology and physics....

Having been to Germany and seeing an experienced bar wench carrying ... 24 1-liter steins of beer sans tray (they only use their hands and what God gave em), the chest size is for balance and support. Trust me, she wasn't pretty, well she might have been 30 years ago, but she could definitely carry beer... Thank you ta-tas...
See, that makes no sense to me. If her large bosom was on her back, or perhaps less disturbing, if she carried the steins behind her back, then I would say balance has something to do with it. With everything in the front I would be curious to see how large their feet are since the barmaids don't just toppple over.

Even if you could see him, have you ever seen a dwarf willingly let go of a full glass of drink? Makes for a really poor bartender.

The bosom is used to support/cradle the 24 steins...

Not balance. Support.
