Winning Combinations

I convert an AoO to twin strike, which lets me make 2 basic attacks.
The two attacks from Twin Strike are not basic attacks.

The way the combo shakes out is this:

You're holding a heavy blade (or two). You get handed an OA. You use HBO to substitute Twin Strike for the basic attack of the OA. You make both attacks. If you hit with the Primary attack, you also get to make a second attack with your off-hand weapon at -5. The end.

Now, you could argue that, if the off-hand weapon is also a heavy blade, that second attack is also an OA, and could be converted to Twin Strike. You'd be wrong, because it's /specifically/ an attack with the off-hand, and Twin Strike requires both hands, but you could argue it. Perhaps, you could convert it to some other at-will, to make it a little cooler (Careful Attack, maybe, to partially counter the -5).

Make sure your Heavy Blades are Scimitars, too, so you can have the DEX damage from Scimitar Dance when you miss.

I don't see the discrepancy. I convert an AoO to twin strike, which lets me make 2 basic attacks.

No, it does not. Twin Strike attacks are not basic attacks. Check the bolded text here.

Heavy Blade Opportunity
Prerequisites: Str 15, Dex 15
Benefit: When you make an opportunity attack with a heavy blade, you can use an at-will attack that has the weapon keyword instead of a basic attack.

Those 2 would be considered AoOs as well, which in turn lets me replace those 2 basic attacks with twin strikes of my own.

Incorrect. Since you used HBO to replace the single basic attack of the OA with an at will power, you can no longer use HBO to replace those two attacks with an at will power each (since they are not basic attacks).

Nor can you use TWF on the off hand attack.

Two-Weapon Flurry
Prerequisites: Dex 19, Two-Weapon Fighting
Benefit: While holding a melee weapon in each hand, if you make a successful opportunity attack with your primary weapon, you can also make an opportunity attack with your off-hand weapon against the same target (but with a –5 penalty to the attack roll).

You can use TWF on the primary weapon to get an additional off hand attack with the off hand weapon, but only if the primary attack hits.

Now I have 2 main-hand and 2 off-hand attacks, each of which I can replace with twin strikes.

But as illustrated, you cannot do this according to the rules. Hence, the rest of your assumptions do not work.

The only combo here is:

1) Use HBO to replace OA with Twin Strike.
2) Attack with primary weapon.
3) Attack with secondary weapon.
4) If #2 hits, use TWF to attack with off hand weapon at -5.

No more, no less.

HBO will only replace basic attacks, not attacks from at will powers.

TWF can only be used to add an off hand attack if the primary weapon hits.

The error people make here is to assume that they can use HBO to replace any of the off hand attacks with an at-will attack. However since HBO only replaces basic attacks, the combo does not work.
