The Monster Manual clearly points out that all types of Horses and Mules are Large animals. Only Ponies are medium animals, small horses under 5 feet tall at the shoulder, and they're mostly ridden by Small Humanoids.
 

I? I'm a teenager (13 years old). Less than five feet tall. But the only time I rode a horse (NOT a pony) it was 5'6" at the very least - and that just at the shoulder- and closer, I think, to 6' or 6' 3". It certainly was as tall or taller than many -if not most - adult women and quite a few men. And some breeds, like perchero, are easily 6' at the shoulder. That, I admit, is a Large animal. But horses (even without considering ponies) range from dog-sized (miniature horses - Small) to Percherons (Large), including some Medium breeds. Just, the more commonly used ones in the medieval/fantasy world are the Large breeds (they're sronger).
