I'd say stick with Con 10 and just give them HD 1/2.

I'd recommend 1/4 HD, or even 1/8 HD, as Tiny vipers and normal rats have only 1/4 HD, and I can't imagine these itty bitty frogs having more.

CR 1/3 or even 1/2? Tiny vipers are 1/3, and their poison is less deadly.

I think that's a legacy of 3e. No such limit anymore.

I increased the racial bonus on Jump checks to +12 to offset the -12 due to its slow speed.

Updated Homebrews. Anything left?
