Rolecall for the Dead - Bring out your dead!

I saw the 'Interpreter' a couple of weeks ago and kinda liked it. One of the more poignant scenes was the reading of a sort of 'rolecall for the dead', naming the victims of a tragic civil war in fictional Mobotu and what killed them. Like "Tom Smith, shot to death", just on and on for hundreds of pages...

It occurred to me that my campaign of two years has accumulated a few PC deaths, some interesting and heroic (well, one), most not. But I'm always fascinated by stories of character death, so if you would follow suit with your current campaign:

Name forgotten, speared to death by kobolds in Ochero
Zebediah, speared to death by kobolds on the LightTower plains
Rupert, cut in two by the demon barbarian while defending the town of Kalos
Lars, shot to death by kobold scouts in the wild
Torg, drowned in several million gallons of liquified kobold remains
Taylith, drowned in several million gallons of liquified kobold remains
Kyle, drowned in several million gallons of liquified kobold remains

Heh heh... forgot how lethal the kobolds were.

Darius Kord slain by a troll and her bodyguards and then eaten
Ormand the Redd slain by bodyguards
Diedre slain by troll
Farned slain by troll
Houshang slain by troll
Pax slain by half orc assassin
Exler slain in ambush with half orcs
Rogg slain by gnolls

edit: also 8 other NPCs along for the ride with the PCs as troops under their command slain by owlbears
and nearly 20 other soliders poisoned by one of the PCs including the party's former post commander Sgt Guido

Only including permdeaths here:

Name Unknown: a cleric slain by ghouls in his very first adventure ever.

Dirk Starsearcher: Slain by a blue dragon after being level drained by a wraith. PC was a bit of a rotten apple, so he was not missed.

Sir Toril: paladin killed by Githyanki, sacrificed himself so that the rest of the party could make it back to the prime.

J'rrick: Half-drow bard, slain by ghouls when he ventured into a dungeon by himself. I've said it before: any monster that paralyzes is death to a lone adventurer.

Laeric Silvermane: human fighter. Killed by his fellow adventurers because his evil nature was increasingly disruptive to the party. This one did come back, but as a villain.

Esse, sahaugin monk/rogue/drunken master: Brain exploded by an empowered mind blast from a grieving widow whose latent psionic powers were nurtured by the Bad Guy. Too bad he wasn't actually the party member who killed her husband. Unlucky guy.

Cornelius van Hauptenschplite, human warlock. Killed by his son after refusing to fight family. Of course, he was a lich at that point, even if he didn't realize it at the time.

Name I can't remember, goliath barbarian. Jumping over a pool of water and missing your Jump check by 1 is bad. Finding out that said pool contains a gelatinous cube and failing your Fort save is worse.

Kiryana - Falling from a 100 foot tower while being eaten alive by hellwasp swarms, and dying from poison in her veins. My first (and only) PC death.

In the campaign I'm currently DMing:

Torell Steelsmith: Killed by a blue dragon while his fellow party members abandoned him to loot the dragon's horde.
Lyle Quickleaf: Killed by another PC. Note that these two deaths were from the campaign prologue, using Pregens that included evil characters and without any reason for the PCs to be loyal to each other.
Flix the Halfling Rogue: Assassinated while playing politics.
Seldszar the Drow Rogue: Killed by a flock of fiendish dire bats.
Rudyard the Ranger: Decapitated by the Lady of Blood, a half-fiend drow BBEG.
Fnipper the Deep Gnome: Killed by an insane fellow party member.
Tebryn the drow rogue/wizard: Killed after going insane by own party.
Viet the dwarf psion: Killed by a half-machine deep gnome rogue.
Zethar the Ghaele Eldarin: Ambushed by rogues.
Keth the Half-Dragon: Killed when a two-headed dragon exploded.
Scyte the Cleric: Surrounded by Noble Salamander rogues.
Err the Half-Ogre/Half Dragon: Killed by a Noble Salamander ranger BBEG.
Azat: Killed by a minotaur death knight (Thanks Blackdirge!)
