My "campaign" has been continuously run since 1980, but I'll only count the 3 current groups I actively run.

Otto, Half-Ogre Fighter 5 - Slain in combat with a Lich. Animated as a large zombie by the lich in the same combat, then destroyed by a sun domain powered turn dead attempt....oh dear.

Lucius and Harold, Human Fighter 1's - Nominally NPC played by 2 of 3 players who happend to be at the house when the game was run. Slain by Orcs. Roger, the 3rd of these, survived by having Toughness, Toughness and Improved Toughness as his 3 feats. The party has kept him around ever since.

Timur, Human Monk 3 - Slain by an Orc Adept, the party was gambling over who would slay the most orcs, and a party member used an attack of opprotunity to trip another party member while he was charging the adept so as to keep the wager alive, the adept lived through the round, and was able to attack the Monk, slaying him.

Donald, Human Fighter 4 - Slain by another party member (a Marshall) who blamed it on a Beholder (Shackled City is fun!). His cousin is another party member, and has vowed revenge upon the beast for the death.

Yeege Yuk, killed by a some damn robot or another
Griffon Highwater, eaten by rot grubs
Ulrik of Garagos, sneak attacked by the party rogue
Romulus Bloodbane, slain by a babau demon...
...slain by a white (?) dragon
...slain by a yochlol
...and finally slain by a demon in the presence of Lolth Herself.

Current Capaign has only seen one death so far:

Flavious Maternus Suburana: (Paladin/Cleric) Rended to shreds by a Troll after charging into the room where the Troll slept.

Kiryana - Falling from a 100 foot tower while being eaten alive by hellwasp swarms, and dying from poison in her veins. My first (and only) PC death.

Well this counts for three.

Bernadette LeCroix, cat-loving wizard, captured by drow, sold to drow, died of starvation. Raised, and captured again ...
Amy, Bernie's cat familiar, eaten by ogre
Veridian Turnlover, electrocuted by blue dragon
Alya Silverleaf, shredded by a gorillon
Alya Silverleaf, smashed by a Maur
Arendel, rended by a shapechanged druid

Shae Wildheart - posioned by Snake conjured by ghoul king.
Samuel di ' Barra - paralized and killed by lieutant of ghoul king
Anne and Eric Whitetree, died in each others arms, trying to escape from Kaorti Sorcerers.
Richard Brightsun, killed by zombie Ogres after multiple botched turn attempts.
Barbarian, name forgotten, sufficated in bag of holding, then lost in rupture.
Ursus the Archdruid, sufficated in bag of holding, then lost in rupture.
Halfling rogue, name forgotten, beheaded by scythe trap in Sunless Citadel.
Marvin the Paladin killed by goblins, Sunless Citadel.

thats about 3 campaigns worth.

so many names & deaths I forget...

A Kara-tur character that took me 2 hours to make. Died in first round of his first battle from the breath of a black dragon.
Ghost the theif: Met some lava elementals. Ate Lava.
Ranger who's name I forget: Slain defending party from attacking dire wolf.
Dirk the Wizard: Made some witty snide comment to a Vampire. The vampire's right-hand man threw a fireball at him. He did try and dive through a window to escape but to no avail.
Feng the Half-Orc Barbarian: Hold Person. Coup-de grace. 70+ hit points did not save him.
