We're only talking print products here, right?

Doesn't that exclude Murchad's Legacy?

If we're not only talking print products, there's a huge number og settings not listed yet - just check rpgnow d20 settings.

Murchad's Legacy was available in print at one time.

Pirate's Guide to Freeport (2007)

This one does contain the continent beyond Freeport, and one could argue that Freeport itself is a setting (you could run an entire campaign with it, given the wealth of adventure material).

This link may help in finding info. It's called the TSR Archive, but it has pretty much every D&D product all the way through 3E including most D20 3pp products. It's not just campaign settings but it could be scoured for anything that has been missed. http://home.flash.net/~brenfrow/
