I'm not sure on the grapple question. I've never thought about it.

The grapple is initiated with a touch attack attack, so I guess it would take the penalty associated with the hand in question. Is that correct?

That would imply an unarmed character could make two penalized grapple touch attacks with twf penalties, rather than a single one with no penalty. Is that correct?

It's not very good, no, but if you're planning to have that hand free anyway....

Technically, the only requirement is that you have nothing in that hand.
The maneuver descriptions do, however, specify what you are doing with that hand, so no unarmed offhand attacks if you follow the description to the letter.

In hindsight, maybe not such a good suggestion....
