It would require a fair amount of hacking, but Mage: The Ascension has a lot of tonal fit with Planescape (factions jockeying for their philosophy to take control of reality)

It's a tonal match for Paranoia as well, for similar reasons. Everyone's in one or another of a collection of wacky cults, it takes place in a weird dystopia, the bizarre entity that leads the dystopia has forbidden people from worshipping it as a deity

I'd always wanted to try the Pendragon system with Greyhawk or Birthright.

Using the new Aliens ruleset for Ravenloft would be terrifying!

I think Savage Worlds would fit the MTG worlds like Ravnica or Theros very nicely.

DSA ( the black eye) could surely be used for Ravenloft w/o much change. Magic and divine would be different but that is not essential for ravenloft.
With heavy modding for psionics it could eventually be used for darksun, but it would definitely make the setting being felt differently.

I would use Cortex Prime for Planescape: core traits would be Allegiances (Alignment and Faction, a la d20 Modern), (class) Roles, and Approaches.

Barbarians of Lemuria for Dark Sun.

I don't have strong opinions on the "default" settings, and the others settings I care about-- Spelljammer and Mystara-- are so quintessentially D&D I can't imagine them being played in a different system.

I haven't tried this, but I would quite happily use Mythras for a Theros game, or for a more low-magic game in any generic setting. For more lyrical or saga feeling games I'd probably use a modified Dungeon World rules set.
I could see Mythras working for the Nentir Vale as well.

Forgotten Realms seems to have a lot of famous characters of very disparate power levels, so maybe a Cortex type game? You could have Elminster, Drizzt and Volo in the same party.

Masks (preferably M&M, if it's not too D20 for the OP) or some other superhero systems where characters of very different power levels exist and interact seem like good options for FR.

For Dark Sun, Moddiphius' Conan 2d20 game seems suitable. It would be even lower magic than D&D Dark Sun, but in many ways that works even better for a campaign where the Kings and their direct lackeys are the main spell-users.

For Birthright, I'd adapt Legend of the Five Rings. It could lead to a game focused more on court activity rather than adventuring, but I think my players would enjoy that as an option.

Eberron seems like it would be a great fit for Savage Worlds, but I'd be inclined to give a try with Airship Pirates.

For The Known World/Mystara, I'd get Barbarians of Lemuria out. It's got space for some of the zanier aspects of tKW while still being fairly deadly and allowing a variety of character archetypes.

If you go back to when TSR had Lankhmar, that's a much better fit for Mythras or another BRP game than it ever was for D&D.

Any thoughts on the World Axis/PoLand?
I did propose either Exalted or Mythras. From what I recall, Sundered World for Dungeon World came out of a gonzo post-apocalyptic 4e homebrew setting based on the PoL setting with touches of Spelljammer. I could likewise see Dungeon World working for Nentir Vale.

Depending on what you wanted to emphasise in it, Pendragon/Paladin might work. The former would probably work better as some editions include PC magic users and isn't as focused on Christian knights. It would work quite well imo for a party trying to expand "civilisation" into the wilderness, establishing themselves as lords of a territory against native opposition.
