Yeah, in Limited, it's "situationally useful" at best, but it is not likely to come up too often. In Commander, as @Charlaquin says, this is a major fat bomb.
I can't see the lifegain effect triggering much, even in Commander. You might be able to pay 11 mana, but 8 cards in your graveyard with different mana values? How often does that happen? Even a graveyard-centric deck will have to work to hit that threshold. And a graveyard-centric deck would probably rather keep those cards in the graveyard.

The base recursion effect is decent, though.

In low level arena I've wom duels using only Order of Midnight and a single Liliana for recursion. This card puts them to shame, and lets not get started on BG decks which had a lot of new toys last expansion.

When ~ ETB, if you've not completed Tomb of Annihilation, return ~ to its owner's hand and venture into the dungeon.

Whenever ~ attacks, for each opponent, you create a 2/2 black Zombie creature token unless that player sacrifices a creature.
