The most famous secret menu is, undoubtedly, the one at In n Out Burger.
Note that the official "not so secret" menu doesn't contain a lot of the popular "secret menu" items, like Animal Style fries. If you go to "hack the menu" they have a lot of different "secret" menus, which are really just various menu hacks.
A famous one, for a while, was the McGa....Ba... (NSFW term), which was a McDouble and McChiken that are combined.
Some people probably just needed stuff to do while waiting hours for their chicken sandwiches, I guess.
