Is there a History of how the Phylactery and Liches came to be?

Interestingly enough there is Kostchtchie from the FR. Not the exact same spelling, but close enough that I wonder if there is a connection there between inspiration for the Demon Lord, or if it is even more.

I have a lot of Dragon Issues, not sure if I have that one (that's a later issue I think. I have all the TSR ones, and many of the ones that came out by Paizo/WotC, but not sure if I have that one...I'll have to look).
