Put the weird and undesirable things like beasts and undead at the bottom end of the d100 table, and add some weird-but-desirable things like celestials at ranges above 100, then start the spell out at low level and allow players to add a certain amount to their roll for each level you upcast it.

Most of them do actually make the distinction. Either you're reincarnated into a newborn body, or you are physically transported in your own body. Obviously a bit of a tougher distinction for inanimate objects.

I never managed to get into the anime because the art style just really doesn't work for me. Just something about the general character design that I can't get comfortable watching. Enjoyed the first live-action season, will be checking out the second soon.
