[Update Version 2.0 5/13/2015]
- Cleaned up some graphical glitches
- Changed form fill font to be the same font throughout and more consistent in size.
- Removed the gray background from top section
- Changed to 150 PPI instead of 300 PPI to reduce file size
- Added Version number to bottom of document
