    We usually just pick our age or roll them up according to the PH. As I get older though, I find it tougher to make the starting age of my character a lot younger than myself. So a lot more of my characters start out being in their mid-twenties, as opposed to being teenagers.

    Yes, I allow placement of spells because I think that taking that away from spellcasters only serves to make the game frustrating for both myself and the players.

    In general, I'm fairly lenient when it comes to this. I allow some table talk as far as flanking, spell placement, etc. However, I do have my limits and it usually involves the pacing of the battle and the game. If players table talk for a minute or longer, my tolerance for it grows shorter...

    My DM did the same thing with "The Fog" for a Masque of the Red Death campaign. It's funny, but suddenly realizing the inspiration for an adventure right when crap starts to hit the fan actually makes the game more scary or exciting, IMO.

    Confirmed - Rangers get d8 HD in 3.5e.

    +5 against a species is inconsequential considering the character would have to be a 20th level ranger. Other characters are dealing out far more damage through feats, spells, or abilities like sneak attack. I think the favored enemy ability needs to be enhanced to make the class worthwhile.
