I've used it both as DM and player and while it isn't perfect, it works very well. Combine it with Dungeon Crafter or some similar map-making program and you don't even need a scanner. Skype also really helps so you have voice chat.

Skype plus Fantasy Grounds is an excellent combination. You use FG for the maps, visuals, and die rolls, and can speak in real time through Skype.
