    Ditto, I think I have 2 9th level characters in my 4-year? long 3.5 campaign. Heroic Tier for me, although the Paragon and Epic rules can be used for building villains like crazy Black Company wizards, insane godlings, etc.

    That blacksmith had always been a hero, he just never knew it until forced into it by circumstances. Ordinary people that survive multiple gunshots, failed parachutes or being lost in the desert? They're just PCs that never took up adventuring yet.
