Ditto, I think I have 2 9th level characters in my 4-year? long 3.5 campaign. Heroic Tier for me, although the Paragon and Epic rules can be used for building villains like crazy Black Company wizards, insane godlings, etc.

I would dig the orcs a lot more without the giant hands.

I replaced animal companions IMC for the same reasons....every "big" fight involved the huge crocodile eating the villain in 3 rounds.

I dunno, I would have no problem having a tribe of giants who serve dryad mistresses who ride them into battle IMC, makes me think of vengeful spirits of the wilderness. I've never been a stickler about following the "recomendations" regarding alignment, ecology in the official source either...
