IIRC, the Istivin Trilogy in Dungeon Magazine (around 118 I think, but I could be wrong) is pitched about Level 11.
From Necromancer Games, there are the following high-level modules:
The Diamond Fortress (Level 11-13)
Fane of the Witch King (Level 10+)
Chaos Rising (Level 12+)
I'm not sure whether thes are 3E or 3.5E, but I ran The Grey Citadel in 3.5E when it had been written in 3E with very few problems (if any, tbh).
There's also the mega-campaign Necropolis by Gary Gygax, written in 3E for levels 10-18. It's Egyption-themed, a series of nasty dungeon crawls laden with traps and demons. I've read chunks of it and it seems challenging (i.e. deadly). I think there are a few issues with errata and some issues with the mechanics (ported from another system) but these shouldn't be too hard to iron out.
