Nezrak managed to land his blade in a weak part of the centipede's carapace (16 +1 =17). But he did not wound the creature with his sword (2 +1 -5 =0). The elf suspected that there was some supernatural toughness afoot because he knew that his blade had struck true.

Rasereit's turn

OOG: I had assumed that Rillian already had his short sword drawn. Chances are though that he wouldn't be able to flank and hit this round, just one or the other (move into position or attack).

Axes already drawn, Rasereit darts at the new foes, weapons flying. "Go fer the big 'un, mates! Focus yer attacks!" With a mighty dwarven warcry, the stalwart barbarian chops away at the giant critter.
 

Rasereit's swing was not able to penetrate the carapace of the creature (Attack Roll: 6 +4 =10)

Rillian moved into a flanking position, looking for an opportunity to stab at the centipede.

Ogrin attacks with his rapier after manuevering a few steps to get into good position for flanking.

Scun circuited the stairwell as quickly as his restrictive armor allowed him (G24). While the battle slipped out of his line of sight, the sounds of ringing steel and chittering carapace did not fade from his senses.

Mona stayed where she was. She would delay her response until she saw when and where her healing was needed.

The other halfling in the party found a position flanking with Rillian. He then thrust at the centipede but found his reach lacking for the moment (Attack: 3 +6 =9)

The centipede continued its attack on the smallest of the party it could reach, having seen no threat as of yet from the other members. It missed Ogrin once more, its mandibles catching only empty air. (Attack: 7 +5 =12)

The other centipedes finally made their move. The eight Tiny centipedes split into pairs. The vermin scuttled underfoot (taking a 5 foot step) and bit at their enemies' feet.

As the vermine approached, Rillian impaled one on his blade (Attack: 14 +4 =18, Damage: 5 +3 =8). The remaining centipede was not able to pierce his boot leather. (Attack: 11 +4 =15).

Nezrak was not able to stop any of the vermin attacking him (Attack: 11 +2 =13). Of those vermin fighting beneath Nezrak, only one was able to prick the wizard. (Attack1: 18 +4 =22, 1 damage, Attack2: 2 +4 =6). Like Rillian, Nezrak's fortitude prevailed against the bite's effects (14 +1 =15).

Rasereit managed to hack and splatter one of the centipedes swarming him (Attack: 19 +3 =21, Damage 11). The remaining centipede drew dwarf blood with its mandibles. (Attack1: 14 +4 =18, 1 damage, Attack 2: 7 +4 =11). His dwarven resistance to poison made little of the threat of centipede venom (12 +6 =18)

The last pair of centipedes were deprived of one of their number as Ogrin skewered one of them with his rapier (Attack: 17 +4 =21, Damage 3 +2 =5). Only a single centipede amongst the pair was made it past the halfling's stabs. But it was unable to damage Ogrin. (Attack2: 8 +4 =12).

ooc: I just wanted to check a couple of things here Nimisgod:
First, which squares did everyone just end up in (including centipedes) ?

Second, when a tiny creature enters a square occupied by an opponent, it provokes an AoO, this is in addition to any AoOs provoked by moving out of a threatened square. So those centipedes should provoke an AoO for entering our squares, even though they only took 5ft moves.

Third, if I attack a tiny creature in Nezrak's square, does it count as flanking with Nezrak?

Nezrak's blade clanged ineffectively upon the centipede's carapace. (Attack: 2 +2 =4)

oog1: adding map now. I couldn't add it earlier since I was away from my computer.

oog2: I had overlooked this, sorry. I had assumed that the AoO mentioned was from movement (ie. leaving squares). Fixing it now.

oog3: I can't find precedent of this in the rules. My ruling for now is "No". fortunately, however, soft cover doesn't inhibit melee attack rolls.
