Does "aid another" in combat stack? Could 15 rats 'aid another' to their giant rat leader and effectively give a +30 to his attack?

Keep in mind, of course, that rats aren't intelligent enough to aid another in combat. nothing with an intelligence of 3 or lower would be able to perform such an act.

Isn't there a limit on the # of creatures that can aid another? I know there is a limit to the # of creatures that can grapple a single opponent...

Well all the creatures aiding must also be threatening the creature that is being attacked, so assuming a 5' reach, that's a limit of 7 aiders.

The description supplies the limit, both the aider and the aided person must threaten the enemy. (at least in 3.0).

Sooo... depending on the size, you can put different numbers of creatures next to someone ...

7 medium aiders you mean... more for smaller creatures.

Nope, not all the medium aiders could touch him AND threaten the opponent.

Edit: Looks like I'm wrong, you don't need to be adjacent (anymore?) to your buddy to aid him.
