How far do you deviate from the "core rules" in your campaign?

Not that much, really. I use plenty of campaign specific stuff, but other than that I keep thing pretty "clean". And by "campaign specific stuff" I mean a several books filled with prestige classes, new spells, magical items, three large monster books and a few campaign specific rules changes, such as:

The Curse of Mesos (Relics and Rituals 2)
The Slarecian Legacy (Relics and Rituals 2)
Arcane Heat (Relics and Rituals)

There are also a whole lot of variants available to me, but I usually don't bother with them, since most of the guys & galls I game with aren't hardcore enough to have mastered even the PHB 3.5. Let alone scores of other accessories/supplements/resourcebooks/variant rulebooks I happen to keep in my bookshelf...
 

Wait, I'm a little unclear on what you're asking (based on your poll choices)- is it rules I've changed or options from other material that you're talking about?

In other words, if I use the feats, prestige classes and stuff from the WotC 'non-core' books, but mostly run things by the book, do I choose the "massively house rule" answer or the "a few options" answer? I'm somewhere in between...

I think there should be an intermediate possibility: many DMs have between 10 and 20 HR, but that's hardly saying "massively HRing".

About myself, in general I like playing with the rules as written except that I don't use multiclassing penalties anymore and I sometimes (although that is circumstance-dependent) bend the spells effects outside the official limits, when it makes for a nice cinematic or rewards the players for inventive ideas :)
 

Mostly the rules as written (a couple of minor house rules) and a BUNCH of stuff from supplemental books. Prestige Classes and Feats mostly, but also monsters, spells and other material.

Oh, and the new Classes from books like XPH, MHb and CW.

I'm not really sure how to vote on this. In the previous campaign I was running, I had a ton of house rules and I eventually realized it wasn't worth it. A lot of times they just didn't impact the game. I'm running the Adventure Path now and it's back to basics.

I am working on a Grim Tales campaign too which is a completely different set of rules and house rules.

I guess for "standard" 3ed right now, I'd vote for #2.

Can't answer. You really need something between the first and the second. I run core 3.5, with no prestige classes or spells from outside, but I do have a fair amount of additions and modifications.
