Been away for a while - back with a question.

Random idea using Sleep as an example:
Normally, Sleep would be an "attack", and the enemy would get a save every round. There's a battle going on around him, and his adrenaline's pumping, and all this other stuff. If it's just a guy somewhere else, though, and especially if he's not expecting it, then it actually sends him to sleep. Out of combat, it works differently.

Tied to this:
I remember reading something about the Succubus. The idea was that the Succubus was taking control of a king. She was using her kiss to do it. On the stat block, the Kiss only dominates for a turn. Outside of combat, though, on an unsuspecting target, it could do more.

You could always go with these ideas.

In my game, I give the players "Spiffy Points" which can be used to do something interesting and unique with a power, both in and out of combat. One example would be allowing a Swordmage to use Lightning Lure like a Whip to pull himself up somewhere or swing on something.
