I'm a firm believer in telling the story that makes sense.
The Three Great Cities: I have four great cities that serve as the largest marketplaces in the universe. Sigil, the City of Doors (which for me is in the Astral Plane), The City of Iron in Dis (Hell), The City of Gold (Heavens), and the City of Brass in the Elemental Realms. Baring local legalities, almost anything can be bought and sold there. For example, the Artifact known as the Book of Vile Darkness has been available for sale in Dis for nearly 4000 years. All it costs are 10,000,000 souls. If you want Very Rare and Legendary Gear - these are the places to find them.
Big Cities: If the PCs are visiting a major hub city with hundreds of thousands of residents, thriving ports, and a huge amount of wealth ... there will be stuff for sale - and stuff that is expensive. The rich and elite may be holding auctions to sell powerful magic items on occasion, for example.
However, my world has a lot of homebrew aimed at creating a world that feels less like one big adventuring hook and more comprehensive. To that end, there is a lot of magic on sale that doesn't have a primary combat use. A bit like Eberron, there is magical gear for sale that is used for business or personal life - including vices. For example you might be able to buy the following things in a huge city:
Towns: A town of a thousand people or less would generally not have much for sale, although there could be exceptions. For example, if a powerful wizard set up residence in the town they might sell magic items - which would serve to support the local economy by making it a tourist destination, etc... However, that would be the exception, and not the rule.
In general, in a town this size you might find a couple places that sell potions and a church or other spellcaster that might scribe a few scrolls. There may be individuals with magic items that could be persuaded to sell them, but that would again be an unusual situation.

- A magical doorknob that can be touched to a wall to create a portal to a demi-plane. Portable warehouse.
- A mask that casts Phantasmal Force on you with a duration of an hour which creates an illusion of a vice experience.
- Rings that cast Unseen Servant.
- Non-combat constructs.
- Sending stones that connect to sages at a great library that can do research for you at any time.
