Unique calendars can add a lot, but unless your game pays special attention to holidays or lunar events, such a calendar isn't a necessity. Most games don't use calendars for anything but marking the passage of time, in which case any calendar will do.
2. Do naming conventions matter to you? Should different areas of the campaign world have consistent language naming conventions?
Yes and yes. This is one of those little details that can really bring a setting to life.
3. Is realistic geography important to you? Mountain, desert, river, etc. placement?
Somewhat. I don't like to see a burnign desert next to an arctic tundra, but so long as obvious impossibility is avoided, the lack of faithful adherance to exact science doesn't bother me. I'm not the kind of guy who will criticize you if your mountain ranges don't reflect the realities of tectonic plate movement as we know it on Earth.
