Someone pointed out to me that the CW 16 rule is also found in the 3.0 DMG (page 27- Designing prestige classes).
There is no erratta for this, but none of the classes given as example prestige classes would have been self-disqualifying (like dragon disciple). The section was rewritten in 3.5 (cf page 197 of the 3.5 DMG), and in 3.5 this rule does not exist. In fact, the section on pre-reqs (page 176 of the 3.5 DMG) seems to say that the pre-requisites only have to be satisfied to take the first level of a prestige class.
Dragon Disciple appears in Tome and Blood, but the FAQ doesn't mention its self-disqualifying nature. So either no one noticed the error, or it was never addressed; it may have been handled with "stealth erratta," I suppose (Wizards customer service, the Sage, etc.)
What strikes me as possible (especially if CW 16 is disregarded) is that one could take a level of mystic theurge as a cleric 2/wizard 3; just have a cleric cast Imbue with Spell Ability shortly before you level up, and you qualify for MT. The result is you take your first level of MT a level earlier than you would normally. (cleric 1/wizard 4 would work, too, as would cleric 1/wizard 3/something 1, or cleric 1/wizard 1/paragon human 3, etc., but these depend on the pre-reqs only having to be met for the first level of a prestige class. You need to have 5 HD to get imbued with a second level spell).
I'm sure the Imbue loophole has been noted before- it's just new to me.
Should a character find herself in a position (changed alignment, lost levels, and so on) where she no longer meets the requirements of a prestige class, she loses all special abilities (but not HD, base attack bonus, or base save bonus) gained from levels of the prestige class.
