Insofar as Centaurs using lances, every DM I've ever played under has allowed this if it came up (and I've played many a centaur)... though as a caveat those were 2e/1e games, so, YMMV (though I doubt there'd be much of a problem in 3e with it). It also was more of a first-round sort of maneuver as well. }:)

Another big question re: Centaurs and Charging Lances is:

If the centaur wants Spirited Charge, how many feats will it cost him? (i.e. can a centaur 'ignore' one or both of the pre-reqs, which obviously were not written with a centaur in mind)

It's safe to say, it's House Rules territory, so it would make sense to just make up a different chain of feats for a centaur to get Spirited Charge.
 

What if a centaur is charging with a lance in each hand?

Actually, you bring up a really good point...

Which leads me to wonder...what about a regular medium size humanoid who is mounted and carrying two lances? Technically, if you follow the regular penalties for two-weapon fighting (with a light lance in your off hand or heavy lance if you don't mind the penalties), and you add in a little magical assistance, you can manage a full attack with two lances, each doing double damage (or more, depending on the mood of your inner munchkin at the time).
