Dave got a Spellcraft check for #2. He did not get a Spellcraft check for #1 because it was a spell-like ability, so there were no motions or vocalizations accompanying the casting to identify.

I do not see any reason multiple Spellcraft checks cannot be made. Perhaps someone is not quite familiar with the motions or vocalizations someone makes in casting the spell, but knows the exact spell when seen, or vise versa. The 3.5 Player's Handbook does not say that you have to fail the save to use #3. It says you can use #3 after rolling a save, without mentioning success or failure. Is your version in the errata?

The somewhat related question I have is for multiple Concentration checks. What if you are on a pitching boat (violent motion) while taking damage? Do you have two Concentration checks? It seems to me that the two factors that might break your Concentration should call for a higher DC check (that much more difficult to Concentrate) rather than two checks. If the DCs were both 21, for example, someone with a modifier of +20 could not lose Concentration if two rolls were called for, but might if one roll were called for at a higher DC.

I think they pretty much have to be separate checks, to avoid having a large number of odd houserules (just on this). If you add them together somehow, how do you do that? There are numerous types of concentrations checks with a lot of potential factors involved and I just think it would be way to complicated to come up with a suitable system to aggregate them. Rolling once and applying the roll for each check might be acceptable, but you're likely to roll separately for some cases when they do not occur at the same time (when casting a full round spell for instance).

Hi everyone, I'm back. It looks like people are having fun with Chevy there, so I'll step back in as NG for the first creature after Chevy is defeated. That'll give me time to get caught up on work and let Infiniti finish out what he's doing. Sound good?
