Combat Advantage and Flanking?

I know that flanking causes Combat Advanatge, but does it grant advantage to everyone attacking the target or just the 2 people flanking it?

For example, if a fighter and a paladin are flanking a kobold, and a rogue is 5 squares away, and throws a dagger, does he get CA and thus sneak attack damage?

The sentence "You have combat advantage against an enemy you flank" from p. 4 of the quick-start rules implies that you need to be one of the two people on opposite sides of the opponent. In your example, the dagger-tossing rogue would not have combat advantage.
