Does using combat expertise count as fighting defensively? The description does not specifically say so, but implies that the feat is essentially an improvement on fighting defensively desribed in the players handbook. Basically you are good at fightin defensively so instead of just trading -4 to hit for +2 to AC you can trade up to -5 to hit for an equal amount of AC bonus. Some other feats allow for extra AC bonusses if you are fighting defensively and I just want to make sure I have everyting right.

Read the entry on page 140 PHB, Fighting Defensively as a Standrad Action (3.5):

"This bonus stacks with the AC bonus granted by the Combat Expertise feat." Because both bonuses are dodge bonuses they should stack

No. Fighting defensively is a separate type of action from using combat expertise. A feat that modified "fighting defensively" would not apply to combat expertise.

No, you can do either separately or both at the same time (up to -9/+7). The entry "normal" for the CExp feat is misleading. If you get a bonus when fighting defensively, you must take the fight defensively option with its -4/+2.
