So far, there isn't a lot on my home campaign ban list...

- Unconvertible elements from 3.0 books... (bits from Savage species & Arms and Equipment guide)
- Choice elements from BoED, (Nymphs kiss, for example)
- Greater Mighty Wallop

What does your banhammer squish?

Feral template - Savage Species. If I allowed it I'd change it +4 LA instead of +1.
Abjurant Champion PrC - Complete Mage
Frenzied Berserker PrC - Complete Warrior
The Blasphemy type spells from the PHB would have to be banned if the game was high level.
Wraithstrike I'm not sure about, it's probably not overpowered at the campaign's planned level (4-8).
 


Not EXACTLY a ban, but I recently changed what I felt was an overly-powerful ability of a prestige class.

In the Expedition to the Demonweb Pits there is a 5-level prestige class called demonwrecker arcane. At 5th level you get an ability called perfect breach. Perfect breach lets you automatically overcome the spell resistance of any chaotic evil outsider.

I decided that was too powerful, so I changed it to an extra +10 to the caster level check to overcome SR against CE outsiders. I think it's a pretty generous amount, especially given how difficult it is to boost one's caster level, but I think it's better balanced overall.

Nothing!!

So far my players have not wanted to use anything I might consider banning. Until they do I leave it as open as I can. :D
 

Currently, anything other than material from the PHB, DMG, MM, Unearthed Arcana, Spell Compendium, and SCAP. When I run Northern Crown adventures, that material gets used in place of SCAP.

I have the opposite policy - everything is banned until it is specifically allowed. And the 'allowed list' is fixed at the start of a campaign (it depends heavily on the nature of the campaign for flavour reasons).

As one other poster in the thread does, I have a solid rule that I won't allow anything I don't personally own. Since I'm the only member of my group to own anything outside of the core (oh, and one copy of the Spell Compendium, but I do own that), that's really a non-issue.
