Let's stick with the "demon standard" greater teleport (self plus 50 pounds of objects only) at will.

Getting back to the "giant bite" and swallow/curse...

Improved Grab (Ex): To use this ability, a dire whiner must hit with its bite attack. It can then attempt to start a grapple as a free action without provoking an attack of opportunity. If it wins the grapple check, it establishes a hold and can attempt to swallow the foe the following round.

Oversized Maw (Su?): A dire whiner's maw can extend to a size for greater than its head would suggest. This allows it to swallow a creature of up to its own size, and its bite attack deals damage as if it were a creature two(?) size categories larger. An obtestu can use its bite attack even while assuming another form.

Swallow Whole (Ex): A dire whiner can try to swallow a grabbed opponent of a smaller size than itself by making a successful grapple check. Once inside, the opponent takes xdx+x points of crushing damage plus x points of acid damage per round from the demon's interior. A swallowed creature can cut its way out by using a light slashing or piercing weapon to deal x points of damage to the interior (AC x). Once the creature exits, muscular action closes the hole; another swallowed opponent must cut its own way out. A Large obtestu's interior can hold x Large, x Medium, x Small, x Tiny, or x Diminutive or smaller opponents.


Should we retool the swallow whole a bit to account for extradimensional storage?

Where would the curse fit in?
 

I don't think we need to change swallow whole, just make the number of swallowed critters appropriate.

For the curse, how about a separate ability:

Obtestu's Curse (Su): Any creature that escapes an obtestu's gullet after being swallowed whole is cursed. It takes X penalty.... The curse can be removed only by X magic.

If we want it to spit out, I wrote this ability:

Regurgitate (Ex) As an immediate action, a quelzarn that is not holding a grappled opponent in its mouth can regurgitate a swallowed opponent. The opponent is no longer swallowed whole but takes automatic bite damage and is grappled in the quelzarn’s mouth.

We can drop the Oversized Maw and just build it in to the flavour text.

I was thinking it could swallow a creature a size larger than itself (Huge) and make its interior hold 1 Huge / 4 Large et cetera.

Something like:

Swallow Whole (Ex): A dire whiner can try to swallow a grabbed opponent up to one size category larger than itself by making a successful grapple check. Its jaws and guts have an extradimensional component that allows them to swallow objects far larger than their external dimensions suggest. Once inside, the opponent takes xdx+x points of crushing damage plus x points of acid damage per round from the demon's interior. A swallowed creature can cut its way out by using a light slashing or piercing weapon to deal x points of damage to the interior (AC x). Once the creature exits, muscular action closes the hole; another swallowed opponent must cut its own way out. A Large obtestu's interior can hold 1 Huge, 4 Large, 16 Medium, 64 Small or 256 Tiny or smaller opponents.

I think I would prefer it to be a deliberate action on the Obtertu's part, and give the Obtertu some motivation for using it - such as it makes its victims more vulnerable to its influence.

Something like:

Regurgitate Curse (Su): As an immediate action, an obtertu that is not holding a grappled opponent in its mouth can regurgitate a swallowed opponent. The obtertu can spit the opponent out into a square up to 10 feet away or keep the opponent grappled in its mouth. The regurgitated opponent is no longer swallowed whole, but must succeed at a DC X Will save or suffer the effects of a powerful curse. The saving throw is ?-based.

The obtertu's curse imposes a -Y penalty to the victim's Wisdom and Charisma (minimum 1). In addition, the victim suffers a -Z penalty to all saves and resistance checks (including spell resistance) against the obtertu's attacks, ability checks, skill checks and supernatural powers. An obtertu's curse can be removed by a break enchantment or more powerful spell, but the caster must succeed at a level check against a DC of X.
 


I like the regurgitate curse, though I think the original curse suggestion was more along the lines of making other people detest the victim. The Cha penalty is certainly appropriate, though.

Why did I put a Wisdom penalty in there too? It doesn't make much sense. I Can't recall for sure, but I think I was trying to penalize the victim's will saves, but the penalty on saves vs whining cover that.

Should we cut it out, Cha is probably enough of an ability penalty.
 

I'd rather the curse be the standard -4 to all attack rolls, saving throws, skill and ability checks. That's effectively a -8 to the Charisma score for the purposes that count, so that'd be sufficiently detestable.
