In my group we have the following rule:
Critical fumbles can only happen on the last attack roll of an attack. So let's say you attack 5 creatures with a burst/blast and roll 1 on the 4th attack, you are good. If you had rolled a 1 on the 5th attack you have fumbled in a small or large way.
For the fumble we roll another d20, if it's between 11-20 nothing happens except for a silly description. on a 6-10 something minor occured. On a 2-5 something annoying happened. on a 1 something bad happened.
Minor = your shoe untied, -1 movement
Annoying = status condition = immobilized, dazed, slowed, prone, etc
Bad = your crossbow just came apart, spend the next round or two doing an easy skill challenge to put it back together. (Perception to see the pieces, Acrobatics to pick them up without getting attacked, Thievery to put it together)
