Twice...
I've encountered decks at least twice... The first time was in the first series of 1e games I ever played in. IIRC, I was like level 1-3, and most of the others were a little higher, as I was new. I was too wary to draw from the deck, but a lot of the others did. One PC, I recall, had to fight a minor death. Another tried to help, and got one of his own. Someone got a card that summoned the goddess of death... I can't really remember too much about what happened, but in the end, I think we all survived.
The second time was a "one-off" game. We were higher level, and one PC was dead. We agreed to try to bring them back. The GM ruled that we had to chose how many cards we would draw, and draw them all, dealing the effects one at a time, in order, but that we went one PC at a time... After everyone else had dealt with their effects, we still didn't have any wishes. I knew that something bad would happen to me, so I tokk three cards, and hoped for the best...
I got two wishes, and wished our friend back to life. Then I got imprisoned, and used the second one to get free... then I fearfully turned over the third card...
I got a +4 Shortbow of Shockburst, or somesuch. Shew!
If it hadn't been a one-off, I would have told the dead PC's player "Sorry!", and refused to draw, SO FAR, my luck has held. Then again, I refused to draw from that first deck!
