Seems like you should role some kind of percentile to determine if certain things (like breach) happen since that is an "event" and not related to the skill of the person delivering the baby. Based on the events set-up you could apply modifiers to the DC?

I agree with other that this should be really easy. It would in fact happen on its own even if no helper was around. I'd say DC 5 for a normal birth delivery. If there's a specific problem that you as DM have indicated for the birth, then maybe DC 10 or DC 15 for some hideous complication. If you want to go old-school 1st Ed. style, then also throw in a good chance for the mother to catch some disease, maybe "filth fever" (Heal DC 12 to avoid).

If you don't have the skill, your "help" is merely "flavor text" and the baby will be born naturally anyway, with all the risk that entails.

Nah, I don't think so.

It's a Heal check. Profession (midwife) would certainly grant a synergy bonus on Heal checks made in the birthing room (or as part of prenatal care), but it certainly isn't required to perform a birth.
 

Yeah, profession skills are based on making a living and are normally performed once a week, even though it is really a service so would fall under profession more so than anything else.

Perhaps a new Knowledge skill might be appropriate to provide synergy.

Good to make an "eppie", use Heal to apply it and diplomacy to calm the mother down. Or bluff to convince her that you actually gave her the good stuff to take the edge off (of course there would be a rather large circumstantial penalty on that one - anyone else ever deal with a pregnant woman?) No way I would ever have enough ranks to convnce her of anything during that time :(
