"Difficult space" and flight

The PHB states that flying creatures are not hampered by difficult terrain. But were they only thinking about certain types of difficult terrain? The examples given on page 284 seem only to include hindrances which are no higher than knee height (rubble, shallow bog, etc.)

What about difficult terrain that go much higher ("difficult space"), such as what a web spell may create?

In our group we are considering house ruling those powers that create difficult terrain in three dimensions (web, sudden storm, necrotic web) to affect flying creatures. Especially in spaces with a "low" ceiling.

Some of the problems we are facing are:
1. What is the height of a power like the web spell?
2. Assuming web is 10' high, would it cost more movement for small/medium creatures (completely inside web) to fly straight up out of the web vs huge creatures (half out of the web due to being tall?)
3. Would a dense forest hinder winged flight (like a huge dragon) and magical flight (wizard/fly spell) the same?

Just use the normal difficult terrain features for difficult terrain in the air..

So if i have 10 feet high difficult terrain, an 5 feet creature needs to move threw 1 square difficult terrain, and 10 feet high creature none to get straight up..
