D&D 5E DM's Adventure Log for D&D 5e?

I was looking at the Dungeon Masters Log for AD&D 1e, and while the second page of that log (monster notes, marching order, light sources, treasure information, general session notes) is still incredibly useful, no matter which edition of D&D you're using, the 1st page of the log is... diminished somewhat in utility. As far as GMing goes, in terms of tracking info, there's some additional information about party members that the guide doesn't cover because in 1e (and to a degree 2e), that information would be standardized - in particular saving throw information. (Argueably, this would be the case for D&D 3rd and 4th edition as well)

Has someone done a modified version of the 1st edition Dungeon Masters Log for editions of D&D after AD&D 2e, and in particular 5th edition?

I have several versions I've tweaked over the past year. I use them to track the encounters faced (and XP), treasure found, and any particular notes. I have a section that I use to keep tabs on the characters as well, including passive scores. I keep tweaking it over and over, trying to find a good mix of data that will fit on one page, but I'm never quite satisfied.

My method was just using an excel spreadsheet. Some cells needed to be merged to balance the page, and the passive skills rotated 90 degrees to limit space. I think I might be able to PM you the file, if you're interested.

One of the most cherished items in my box of old D&D stuff is a 1E DM's Log filled in with all the adventures we ran in a campaign from around '83 to '85. The names of the adventures and the players. All the PCs listed there, many of them with DEAD scrawled across the entry. The monsters and treasure. Brings me back.

That's one of the downsides of the move to digital. Not very likely someone will be rummaging around a box in 2041 and come across a DM log or character sheets from a session in 2018.
