Do you limit or ban raise dead in your campaigns? Do you expect to again in 4E?

I make it harder to access, give bigger penalties, and people usually go for it if they can. I might try the sacrifice Idea - and extend it to include reincarnation.
When a player dies, if they make a new character, they start at the lowest living player level. If I think they killed themself on purpose, or they decided to scrap a character after playing more than one session, they make a new character a level BELOW the lowest. Players dont die all that often in my games. so far I've had like 5 deaths in the 7 years I've been playing/running.

I don't limit it at all.

I do kill PCs regularly.

However, my players seem to hate it -- they prefer to make a new PC if an old one dies.

Thus far, no-one in my game has come back from the dead. Gives me a lot of flexibility as a DM. :)

No limitations on it. In my experience, players are too damn cheap to "waste" so much gold just getting someone raised from the dead... unless a party member can actually cast the spell. And even then, most players (again, IME) seem to rather bring in a new character than mess with the effects of coming back from the dead.

Edit: although one of the other PCs in the Epic Module my planescape group is going through currently has received a True Rezz from one of my PCs minions. :D

commentary: its early here, just got home from work... is the above really as badly written as it looks to my sleep-deprived brain?. :confused:

While I do not limit Rezs in my games I have noticed a distinct lack of need for them recently. With the spells Close Wounds and Revivify (both from the Spell Compendium) it is very, very rare for a PC to die now. Close Wounds especially is a must have for any 3rd level divine caster.
