DR: x/Good vs x/Holy

A 10th level Initiate of Pistis Sophia has DR 10/unholy as just an example. I would suspect you would find some beings with 10/holy in the BoVD and possibly the FF. Basically I see the /holy and /unholy as mechanism to add DR which normal evil/good outsiders cant automatically bypass based on their intrinsic nature.

The BoED reference could be a mistake along the same lines as the Ki Strike (Holy) errors.

The BoVD has no creatures or classes with DR x/unholy, and the 3.5 Fiend Folio update doesn't use the word unholy anywhere.

Well, the class in question is from the BOED, the Celstial Mystic (or as my players call it, Mystic Santa). It gains DR 10/unholy (just like the Initiate of Pistis Sophia)

Perhaps it is a mistake, as I couldn't find any other reference to DR x/Unholy or Holy anywhere else in the book. Even the uber-angel Pistis Sophia has DR 20/Evil. It doesn't make much sense that a follower of Pistis Sophia has better DR (since it would take an Unholy weapon - not just an Evil one).

Sounds like an error to me too.
