The "creatures" refers to the target, not the attacker. If you are 60ft away or 10,000ft away and attacking a creature within 30ft of the low wall (separating), then cover applies.

Why would % dice need to be rolled, regardless? If you use the cover variant then if the spellcaster misses by 4 or less, then he hits the cover. What do % dice have to do with it?

Well the % dice is how you measure hit/miss with concealmeant. Must have gotten confused. What is this cover variant?

No: "The attacker can ignore the cover if he’s closer to the obstacle than his target." I suppose he doesn't have to ignore the cover, but he probably will. :)

When talking about cover, it's just +4 cover bonus to AC. The cover variant is in the DMG. If the attacker misses by 4 or less then the cover itself may be hit. If the cover's AC (e.g. a person) is too high to be hit by the actually attack roll, then the cover is missed too. If the cover's AC without Dex/Dodge bonuses would be hit, however, then the cover does not actually provided cover and the original target is hit instead. All this latter part is irrelevant for a wall.

Don't think it's specifically written, but it shouldn't be a problem as a free action. It still lowers your speed unless you know how to walk in that manner.
