ECL for Monster races who advance "By Character Class"?

Yes, as Xarlen said, simply add the character levels to the starting CR.

You can find this rule on DMG p. 102 (under "Tougher Monsters", in bold text).

Theres been threads on Dual lance wielding Centaurs charging opponents for insane amounts of damage (with other characters on thier backs no less- with Mounted Combat and Ride by Attack). Its scary what you can do with a horse thats actually a PC and can wield weapons. I don't want to think about it.

Where does it say that they're always mounted? I don't think they should be considered mounted anymore than a normal human. They just have more legs.
 

They're considered Large, they have the attacking from higher up (Ala mounted combat), and they can charge.

And, a charcter can ride on their back.

Just because they are attacking from higher up doesn't mean their mounted. Just that they're attacking from higher up. And they may not even get that bonus. No other large creature does. It would completely negate the size penalty to hit. They may have a mount (who is definatally mounted) but they shouldn't be able to use lances as if they were mounted. It doesn't work the same way when you're the head instead of the rider.

For example, instead of being pushed of the horse, you break your back. And they don't get to make ride checks to avoid being hit.

for a centaur charging with a heavy lance, I'd definitely give it the 2x dmg!

might not be legal, rules wise, but I'd give it to 'em anyway :)
