Yeah, there's magic (this almost out-of-the box OA except with only humans, and set in feudal Japan). But I'm mostly looking for the medical effects since I want this behavior to be entirely a delusion and thus merit no benefits.

So, basically, he'll be pretty ill and sickly, and look bloated and ruddy? CON drop is a good idea, yes.

To give your good aligned characters a moral Quandary (if you like that sort of thing) and to add new levels of RBDM-ness you might want to consider what has caused his Demented state of mind. Other than he is just insane and needs to be put down like a rabid dog. For something creative other than the tired myth of porphyria...

Ergot poisoning. During the Middle Ages, it was an epidemic. Ergot is a hallucinogenic fungus that grows on rye, wheat and other grains. To an untrained eye, it is almost impossible to detect. The symptoms can include violent hallucination, difficulty breathing, sporadic lapses in consciousness, muscle twitches, numbness. Not only that but symptoms can appear and dissapear or change over time.

That way he 'could' look completely normal as for the majority of the time he eats a normal diet until his symptoms and violent hallucinations push him to drink blood for a while, and his selective memory makes him forget.

And then the grain is spread around the village......

Blood-Borne Taints:

1/ Ill Humour. As we all know, Blood is a Hot Tempestuous Humour, leading unto Rage if the Will should meet opposition. This Quality of rage should also reside well within the spirit of a Villian.

2/ Bad Karma. Since you drink the prana as you imbie the blood, you will (of course) take on some of the karmic debt of your meal. This is why pure virgins are prefered -- better karma. Thus are the villian's wicked, corrupt cohorts safe from his diet.

3/ Blood Brothers. It's well known that if you mix your blood with that of another man, you are blood brothers. You share in his fate (per karma above), but you also share in his identity. Divinations might mistake you for him (especially if he is not available... because you killed him).

4/ Melting Pot. As above, but you also register as a member of pretty much any race you've ever fed on. Sword of Orc Bane? Well, you are what you eat.

In order to subsist only on human blood (or any other blood) you would need to consume 400 liters of blood per day. The reason is that blood is mostly water. That amount is about a bathtub every day. Clearly thats just not possible.

So looking at the technical side the guy needs to have some way to remove the water from the blood and then eat the dry matter (mostly protein) that will be left.
 


While not an expert in the matter (who is?) I have seen plenty of folks with blood in their belly as an RN. IRL you tend to get sick (N/V/D) and would have some truly foul gas. Now that that is out of the way - since he his a fantasy sort of critter looking to transcend, I would suggest that he mix blood with various alchemical substances to balance himself (maybe something like wine, the aforementioned milk, or maybe nectar (soma)). These mixing would be to heighten various qualities of the blood/spirit.

As stated below, the nutritional content of blood is actually pretty poor by volume. If you want reality, the answer I would say is pretty much, you could not live on just blood. Of course this is gaming, you don't need to concern yourself with reality.
