Ever died from suffering an AoO?

In my Out of the Frying Pan campaign one of the PCs was killed from an AoO suffered when running through the monster's threatened area to apply first aid on another dying character.

The other character survived. She was absorbed into a shape-shifting alien monster and had her soul utterly destroyed.

The one I can remember, is the character that was trying to avoid AoOs but forgot about the reach from the Dwarf who was enlarged. I was even nice and asked if that was the way he was going. One hit and a comfirmed critical later, he was near death, until another player reminded me that axes do triple, not double damage on a critical. Needless to say, the player of the now dead character was not pleased.

Rarely do my players take an AoO if they feel they can be hit or hit hard enough to be an issue. However, dozens of fleeing humanoids have died this way ;)

Just remembered a mage that was brought to -2 for attempting to caste a spell allowing for an AoO.

But he didn't die. He fell and Rolled an 18 Bluff to play dead against the Orc Barbarian.

I've had a PC die that way, though it was a bit of a no-win situation. Bear warrior in brown bear form vs. a war troll, no nearby allies on either side. The character was getting sliced to bits by the war troll's superior melee damage output, even with succeeding on all his saves vs. being dazed. His Grapple modifier was excellent, but the troll was the same size category so he couldn't use Improved Grab. In desperation, he attempted to initiate a conventional grapple, knowing that if he could just dodge or survive the AoO, he had a decent chence of holding and pinning the war troll. Instead, the troll hit and rolled maximum damage, taking him to -11.

I have had a character or two dropped unconcious due to attacks of opportunity, but not killed outright.

Last couple of game sessions I have been purposely taking attacks of opportunity in the face. I am a Lawful Good Dwarven cleric and we have been going against frost giants. My AC against them is ridonkulous after you factor in the full plate +3, the shield+3, amulet of natural armor +2 and +4 AC vs giants thing. I take it in the face so that the characters that are less likely to survive a swing do not have to.

I've severely wounded PCs with AoOs, which has led to a death (maybe two). But my players tend to be paranoid about AoOs, and I tend the same way, so we generally avoid risking them.

Never (directly) killed with one or died from one.

Yeah. We had a paladin knocked unconscious fighting a bearded devil (he had the only weapon that could reliably beat its DR). The cleric healed him; he took an AoO standing from prone and was knocked unconscious again. The cleric healed him back to 0 hit points; he attacked from prone taking the -4, did enough damage to kill the devil, and fell unconscious again :D

-Hyp.

I had a first level Ranger take an AoO while trying to drag the downed cleric into a tower with a working door, so we could heal him. I went down, someone else grabbed the cleric, and they left me outside on the front steps and ignored me long enough for me to bleed out. :( Everyone wanted to hit something instead of healing the cleric so he could heal the rest of the group. I was the only one that died that day. :\
Did I mention that before I died, there was enough time to walk out, pick me up, loot my pockets, and drag me inside? But they didn't..

He pulled off a Bluff while at -2 hit pts? Damn, that's good!
