Not yet anyways.

Seemed like I always was the one that ended up setting off the 'mousetraps' for the others to make a break for it. Good thing the monsters didn't have Combat Reflexes.

You must move at least 5 feet both before and after you make your attack in order to utilize the benefits of Spring Attack.

One of the benefits of Spring Attack is the ability to move both before and after the attack.

Your reading would suggest that until you a/ move at least 5 feet before your attack, and b/ move at least 5 feet after your attack, you cannot utilise the benefits of Spring Attack. One of which is the ability to move at least 5 feet before your attack, and move at least 5 feet after your attack.

Even better, the character died from rolling a 1 on the save against massive damage resulting from the AoO, which was triggered by a charge against the BBEG, which most likely would have fallen, if the charge would have hit; and all that, after that very same character already died once in that very same combat, also by rolling a 1 on the save. Tough luck for sure! :p
That's a neat trick, to die twice in the same combat! How did said character return to life in mid-battle?

There's a spell that brings back a character who died in the last couple of rounds... Revivify?

Never been killed by an AOO. I generally manage to avoid them outside of reach situations, especially when low on HP. (Last time I think I intentionally provoked an AOO was to charge my paladin past a bunch of undead to kill the necromancer controlling them. That was one of those beautiful moments. Smite Evil combined with full Power Attack combined with a natural 20, followed by an 18 followed by a maximum damage roll resulted in the necromancer getting dropped to -10 exactly. Got clawed up a bit by his minions in the process, but so worth it.)

Have killed PCs in my games with AoOs from time to time. One was the rogue blowing his Tumble check against a Frost Giant and then resulting AOO being a crit.

Another was Vampire Fighter/Weapon Master of the spiked chain with Hold the Line and an Unholy, Shocking Burst spiked chain. PC charged her, ate an AOO for the charge (due to hold the line) and then another for exiting her first threatened square to close, which ended up being a crit and dropped him. (In retrospect, I don't think he died, just got taken into negatives.)

I'm DMing, and an 8th level party is fighting a Huge, power-attacking Earth Elemental. The cleric decides to charge with his spear. I make sure he knows that the elemental has reach and will get an attack of opportunity. The cleric's player says "I can take it." So the elemental swings...

- Natural 20
- Confirmed critical
- 4d10+32 = 65 HP of damage

The cleric's shattered body splats wetly against the wall some 20' away and slides to the ground.

I've never seen more stunned looks on the faces of my players as I did in that instant.
