IMC we have simply treated Vorpal weapons as landing a lethal wound upon creatures whose heads may be out of reach. This works well for us.

You hit him so hard in the foot that he stumbles and his neck connects with your blade when he falls, instantly decapitating him.

I would say that the combatant is slightly more effective than just pinking him at the toe. After all, how would you justify the fighter killing him at all with just toe plinking?

A giant's to is very sensitive, you know. Its secondary brain is there, as are the lungs, and several main arteries go through the giant's toe. Little known fact, but true.

Since a fighter with a longsword can fight and kill a colossal dragon, I'd say toe-plinking is a time-honored D&D concept
