Agreed Hand of Evil, nothing comes close to your own homebrew, since it reflects your own likes and tastes better than anything anyone else could produce.
As far as best worlds, both for professional quality and originality of ideas:
1) Midnight
2) Kingdoms of Kalamar- its FR without all the wank
3) Codex Arcanis by Paradigm
4) The Hunt: Rise of Evil
5) Scarred Lands- I recognize this is a high-quality setting, its just not the style of game my group and I enjoy.
