A glaive forged from the Horn of a siver dragon, Frost Reaver. the glaive now carries that same name so that the villain who murdered the dragon Frost Reaver hears the weapons name moments before his death.

To be specific, its a dwarven masterwork adamantine dragon craft glaive.

My elf fighter wields two longswords, Sulian (breath of wind) and Brimstone. He didn't name Brimstone; it was named that when he got it, and it's only on loan anyway. He has another longsword he doesn't use much these days named Tasarol (willow branch).

I named them, using a list of Sindarin words from one of the supplemental Tolkien books. I didn't want to name them anything aggressive-sounding like "Heartcleaver" or that sort of thing. I like the idea of the weapons being named something that doesn't sound weapon-like.

My first AD&D character was modelled after the Grey Mouser, and like him, his rapier and dagger had names. Heck if I can remember what they were, though.

Closer to modern times....I'm currently playing a paladin who has a number of named weapons.

Thorn, rapier (roses are a reoccuring theme for this character)
Redeemer, holy quarterstaff
Lumina, holy mace

Earlier in the campaign, he had a greatsword named Soulcrusher, but that name was stolen from the anime Record of Lodoss War.

I'm playing a roguish type in an epic level campaign based on some of the rules from the Book of Immortals. My character has forged an artefact (spelling peculiar to those rules) dagger that becomes more powerful as he overcomes more immortal challenges. Anyway, the relevant point is that it represents a very large part of my character's offensive capabilities. It's name is "Fool's Hope"

My fave is from Dr. Gary Larson.

"Now this end is called the thagomizer, named after the late Thag Simmons."

Here's a weapon I'm working on for a PBP game I'm playing on another forum:

My Lawful Evil Cleric of Hextor wields Garn, an intelligent Flail.
It's basically your standard flail, except for one horrifying distinction. Instead of a steel spiked ball at the end of a chain, there's a human head, penetrated by many spikes. The head belongs to a certain long forgotten tyrant with designs for world domination who made a deal with the wrong god in an effort to be gain eternal life. He was granted eternal life, mind you, just not in the way he wanted. The only way he can truly die and end his eternity of suffering is to be wielded by the ruler of all of humanity.

The only power that my character knows about is that Garn has 10 points of intimidate. It's very convenient that my cleric is also trying his hardest to rule all humanity... or at least destroy all of humanity that won't bow to him.

My character Smirk is a rogue/assassin who named his +1 wounding dagger Creepingdeath. He also had a +2 dagger he named Cutthroat.

My friend's character Cale is a spirit shaman/stormlord who named his battleaxe Stormhewer. I can't remember the weapon's exact properties, but it had some shock capabilities and was pretty powerful.
