Eberron introduces the option of an Eldritch Machine, a single purpose Wish item constructed via a rare and unusual ritual. That would be my first pick.

Anybody else remember the potions of longevity from earlier editions of the game?

I wonder what it would take to recreate that under 3E... It would have to be an elixir, not a potion, since it's not a precise duplicate of a spell effect, but what spells do you think would be required to create it? Wish, perhaps?

Don't mean this to be a hijack; such a magic item would certainly fulfill the requirements.

Get an object of continuous gentle repose, or have a cleric cast it on you once every few days.

IIRC, it was made from Elven blood. In a camaign world which one of my friend was running, elves were extinct because of it.

Although that route doesn't let you keep the same youth and beauty. Youth you get, beauty is a crapshoot. You might wind up as a tall, goofy-looking, curly-haired British man with a scarf fetish and a taste for jelly babies.

Become a lich...

A lich is a gaunt and skeletal humanoid with withered flesh stretched tight across horribly visible bones. Its eyes have long ago been lost to decay, but bright pinpoints of crimson light burn on in the empty sockets.

Lich + PAO. Though there aren't many pretty undeads to morph into, save possibly vampires.

Steal Life is probably still the best option though. You're forever young long as you cast an eighth level spell once per year or so. Even if you miss it you could make up for it later.
