I'd House Rule that in cases like this, you could throw 1+Int Mod per level, which is still retaining a lot of awesomeness. Imagine playing a psion who walks around in a trenchcoat lined with +1 Shocking Burst Shurikens. I know what my next character is!:D

Just a Thought !!!

I was wondering with all the telekinesis shenenigens on the previous posts, if anybody has considered that whatever applies to the Psion's Telekinesis then surely the same would apply to the Wizards or Sorcerors Telekinesis Spell ? So in that instance then that would mean rolling to hit with whatever you are "Hurling" etc...and that would be fair enough but regarding pulling weapons out of hands etc...wouldn't you have to have the Disarm feat ? Surely it would not just fall to a Will save to retain your weapon, as all warriors in general, normally have crappy will saves, if it is a save, why not apply a relex/dex. synergy bonus as the hapless warrior struggles to hold on to his weapon. Any further thoughts ? ;)

No feat needed, because magic is magic.

The fighter would get a will save. If he was holding the item, a strength check might be in order, where the caster uses something like his caster level + spellcasting mod or just a flat strength that is equal to the ammount that can be telekinised as a max lift.

No dex is involved here, as the magic just affects what the spellcaster wants it to affect. If the will save fails, and the item is free (arrows in a quiver) they go flying. Simple solution and not overly strong for the level of magic that Telekinisis is.
