Second, if I was DMing a PoS (love that acronym!) and had to make the call, I wouldn't be so strict. It says at every opportunity. As DM, I would judge that a bar full of higher level characters, a higher level bouncer and an even higher level proprietor didn't really constitute much of an opportunity. But failing to help granny across the street... and then "help" her to the next world as soon as no one was looking would constitute a breech. I'd also be more strict about disrespecting authority figures. For example, in the RDI, meekly announcing one's name at Joe's request would cause loss of PoS abilities. I'd require a PoS to make a big scene about "Nobody tells me what to do! I'll tell you all who I am after I've decorated my blade with your entrails!", etc. Followed by the inevitable challenge to Joe, or to Gort, or to whoever was handy.
Or, there's the old standby, "Rules, eh? Rules? Ok, I'll abide by your rules. My name is Doublewide Buttstomper, and not only can I lick every man in this bar, but before the night is through, I will! Who wants to be first?"
If no one answers the challenge, he doesn't have to immediately initiate carnage, just make a few disparaging comments about the lack of fortitude that the patrons and staff display.
